How to order

To purchase MHS products through the MHS Online Catalog or by downloading an MHS Order Form, follow these four easy steps. For other ordering options, see Other Methods of Ordering below.


1. Check your qualification level.

Some of our materials are sold only to qualified individuals. Qualification levels (a, b, c, or m) are specified for all products in our online catalogue.

A-level products do not require any specific qualifications.

B-level products require that the user has completed graduate-level courses in tests and measurement at a university or has received equivalent documented training.

C-level products require fulfillment of b-level qualifications, and users must have training and/or experience in the use of tests, and must have completed an advanced degree in an appropriate profession (e.g., psychology, psychiatry). Depending on state requirements, membership in a relevant professional organization (e.g., APA) or a state license/certificate in psychology or psychiatry may be necessary.

M-level products can only be purchased for use in a law enforcement agency setting. For more information, please contact

If you are a first-time purchaser and wish to purchase materials that are identified as (B) or (C) level products, you will be required to complete a Purchaser Qualification Form. Please select the apppropriate form below for your geopraphical location. Print the form, complete it, sign it, and then fax it to MHS. All orders for test materials must include the name of a qualified individual who is responsible for the use of the materials ordered, and your signature.

2. Place your Order.

Visit the Online Catalog to browse products and place your order.

Select the appropriate Order Form for your geographical location. Forms for U.S., Canadian, and international customers are provided. Print the form, complete it, sign it, and then fax it to MHS.

3. Fill in your product order.

Be sure to specify item codes, item descriptions, the price of each item, and the quantities. Calculate the subtotal, add any applicable taxes and shipping/handling charges (as specified on the form), and total the cost of the order in the appropriate boxes. MHS requires your Federal Tax Identification or IRS number for all orders being shipped to addresses within the United States only.

All prices are subject to change without notice. Customers are responsible for any applicable duties and taxes. Prepayment by credit card (Visa, MasterCard or American Express) is required for all orders. If prices on your order are incorrect, we reserve the right to adjust the invoice amount by up to 10% unless otherwise stated on your order.


4. Submit your order.

Institutional Purchase Orders:

MHS will accept Institutional Purchase Orders from hospitals, schools, universities, and government institutions, providing that the purchase order:

1. Is typed on an Institutional Purchase Order template;

2. Bears the complete billing and shipping addresses*, including telephone numbers and Federal Taxpayer ID number or IRS Number if order is being shipped to an address within the United States (*note: MHS is unable to ship to PO Boxes except APO/FPO addresses);

3. Itemizes all product names/descriptions, specifies the quantities, and calculates the total cost (MHS will notify the purchaser of any discrepancies before processing the order);

4. Is sent as a hard copy, either by fax or mail; and

5. Bears an authorized signature

Shipping and Handling will be invoiced to the Customer according to rates published in the current MHS catalog.

Within 24 of receipt, MHS will send confirmation by fax or email to the contact indicated on the Purchase Order confirming receipt and processing of the order.
